They are allowed to bring one helpful item with them, like a fire starter or a machete. Other than that, they're on their own. (Well, except for the camera crew and producers.)

Is Naked and Afraid real?

Naked and Afraid is real in the sense that the survivalists don't have any food, shelter, or clothing, and they're expected to survive for three weeks on their own. However, it's worth noting the production crew is literally following them around, and they're never completely alone in the case of an emergency. Not to mention, the footage is edited after the fact. There have also been reports of producers giving one of the survivalists a tampon when she was on her period. So you can decide for yourself if that makes it any less "real."
